COVID-19 Mandir Visit Disclaimer:

 

I affirm that I, as well as all household members, have not been diagnosed with COVID-19 within the past 30 days. I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days.

 

By entering these premises or registering for any services, I on behalf of myself and any minor child/children I bring to the Shri Mangal Mandir (the Mandir), agree to the below:

 

All persons visiting the Mandir acknowledge and understand the contagious nature of Coronavirus/COVID-19. I further acknowledge that the Mandir has put in place preventative measures to reduce the spread of Coronavirus/COVID-19. 

 

I further acknowledge that the Mandir cannot guarantee that I will not become infected with the Coronavirus/COVID-19 disease when participating in the services/darshans offered by the Mandir. Fully understanding the risks, I acknowledge and assume the risk of infection with COVID-19 arising from my being on the Mandir premises and participating in any services/darshans offered by the Mandir. I am following all CDC recommended guidelines relating to Covid-19. I acknowledge that the CDC guidelines may be modified and updated from time to time and I accept full responsibility for familiarizing myself with the most recent updates.

 

I voluntarily seek services/darshans provided by the Mandir and hereby release, waive, discharge the Mandir, its trustees, priests, employees, executive committee members, volunteers from any liability related to COVID-19 which might occur as a result of any act or failure to act by the volunteers and related to my being on the Mandir premises and participating in the services/darshans. I shall indemnify and hold harmless the volunteers from against any and all claims, demands, suits, judgments, losses or expenses of any nature (including without limitation, attorneys' fees, costs, and disbursements), arising out of or relating to, directly or indirectly, the infection of COVID-19 or any other illness or injury relating to my participation in the services/darshan offered by the Mandir. It is my express intent, that this waiver shall bind my heirs, assigns, and legal representatives.

 

This liability waiver and release extends to all the trustees, employees, executive committee members, and volunteers of the Mandir.
